A SAMPLE ACCIDENT REPORT:
Question:
Imagine that you are a safety engineer in a cement factory. There has been a fire accident in the factory and one of the workers has been badly hurt and is in the hospital. Your general Manager has asked you to send him a detailed report on the accident with your recommendations for averting a similar accident in the future. Prepare a report accordingly.

A REPORT ON THE FIRE ACCIDENT

1.TERMS OF REFERENCE:
    You have asked me  to send you a detailed report on the fire accident that took place in our factory
    at 11 p.m. on 14 April 2020 with my recommendations  to avert a similar accident in future through
    your memo dated 15 April 2020.
2. PROCEDURE:
    a. Several visits were made to the fire accident spot.
    b. The employees who were at the site at the time of the accident were interviewed.
    c. A discussion with the chief electrical engineer was made.
    c. Suggestions for averting such accidents were collected from the employees as well as department   
        engineers.
3. FINDINGS:
   a. Due to a short circuit, there was a fire accident around 11p.m. on 14 April 2020. It seems enough precautions and preventive measures had not been taken to provide for safety in such circumstances. The fire spread rapidly burning and damaging several materials kept in the store room which is close to the place where fire broke out.
  b. The electrician, Mr. Raman, who was on duty sustained serious I juries and was rushed to the company hospital. After giving him first aid, he was transported to government head quarters hospital. His condition is critical.
  c. Three more employees on duty, Mr. Gopal, store keeper, Mr. Ashok, store clerk, and Mr. Vijay, night watchman sustained minor injuries and were given first aid.

4. CONCLUSION:
  a. Short circuit was the cause for the accident.
  b. Enough precautions and preventive measures had not been taken.
  c. The security department acted swiftly to attend the persons involved in the accident.
  d. Over heating of electric equipment and the electrical short circuit were the two main reasons for the fire accident.

RECOMMENDATIONS:
   a. Temporary wiring and overloading of equipment should be avoided .Only approved electrical equipment should be used. Proper earthlings and overload protection devices should be installed to prevent electrical faults. Electrical installation should be periodically checked by a competent and certified person
   b. Gas cutting and welding operation should be avoided near areas where highly inflammable or highly combustible materials are stored.
  c. Fire extinguishers must be provided at strategic points.
  d. Proper means of passage by which the people in the building may reach a place of safety  should be provided.
